One item on the official Happy Meal list has no published portion figure in the data set behind McDonald's own nutrition calculator: McFish. It appears in the allergen booklet as a choice, but no per-portion row was retrievable for it, so it is missing from every table here rather than guessed at.
The lightest box and the heaviest box
The 488-calorie gap between the two extremes is the single most useful fact on this page. Parents tend to treat "a Happy Meal" as a fixed cost, roughly 400 calories, and order on autopilot. It is not fixed.

What people actually order
The extremes are useful as boundaries, but almost nobody orders cucumber sticks and still water. These are the builds that actually go over the counter, with the two numbers that matter most next to the calories.

Look at rows four and five. The cheeseburger box with water (535 kcal) and the nugget box with milk (536 kcal) are the same calorie total to within one, and they are not the same meal at all. One carries 2.14 g of salt and nothing to drink but water; the other carries 1.10 g of salt plus 9 g of protein and 125 calories of milk. Identical on a calorie tracker, a long way apart on everything else.
That is the argument for never judging a Happy Meal on its calorie total alone.
What each swap costs in calories
Three choices, three levers, and they are nowhere near equal in size.

The side is the lever. Dropping the fries for a carrot bag takes 203 calories off, more than any change to the main and more than removing the drink entirely. Nothing else on the menu moves the number that far.
The last row is the one worth arguing with. Milk is the most calorific drink in the box, and it is still the pick most likely to leave a child fed rather than just watered, because those 125 calories arrive with 9 g of protein and no free sugars at all.

Two veggie dippers are 160 kcal and four nuggets are 174, and there the resemblance stops. The nuggets carry 10.0 g of protein against 3.3 g, and they do it on the lowest salt figure of any main in the box, 0.38 g. If the vegetarian choice is being made for the child's diet rather than for ethics, it is not the lighter option; it is the lower-protein one.
The step from hamburger to cheeseburger looks small on calories, 255 kcal to 298 kcal. On salt it is not: a single slice of processed cheese moves the main from 1.1 g to 1.7 g before the fries are added.

The spread here is nearly twenty-fold, from 12 calories to 237. Small Fries cost more than all five of the other sides added together.
Two details that get missed. The carrot bag carries 2.4 g of fibre against the fries' 2.6 g, almost the same amount, for 34 calories instead of 237. And the fries are the lowest-sugar side on the list at 0.4 g, which is a reminder that "low sugar" and "low calorie" are unrelated claims.
If you make one change to a Happy Meal and no others, make it this one.
The drink is where the sugar sits

On calories alone the milk looks like the worst drink in the box and the orange juice looks reasonable, 25 calories lighter. Read the sugars column and it inverts.
The 22 g in the carton of orange juice are free sugars. Sugars released from fruit during juicing count as free sugars, which is exactly the category the NHS advice on cutting down on sugar is about. Against the UK government's daily free sugars figure for a 4 to 6 year old, 20 g for boys and 18 g for girls, one carton is 110% and 122% respectively. The drink alone clears a young child's entire day.
The 12 g in the milk are lactose. They are not free sugars, they come with 9 g of protein, and they cost 25 calories more than the juice. On this menu, milk against orange juice is not a close call.
The Fruit Shoot at 8 calories is the anomaly: sweetened, but at 1.6 g of sugar it behaves more like water than like juice on both counts.
Salt is closer to the limit than calories
This is the number most parents never see. The heaviest box, cheeseburger with Small Fries and milk, carries 2.42 g of salt. The UK government's daily maximum for a child aged 4 to 6 is 3 g, so one meal is 81% of the day's allowance.
Compare that with the energy figure for the same box: 44.5% of a 4 to 6 year-old boy's daily calories. The salt is almost twice as far up its ceiling as the calories are up theirs.
At the other end, the lowest-salt build available is McNuggets x4 with cucumber sticks and water, at 0.39 g, or 13% of the same limit. That is a six-fold range, and it moves independently of the calorie range: the cheeseburger and the cheese slice drive the salt, not the fries.
The NHS guidance on salt sets 6 g a day for adults and lower figures for every age of child, which is why a box that looks modest against an adult label can be most of a small child's day.
How much of a day this is for a child
The UK government dietary recommendations set estimated daily energy at 1,482 kcal for a boy aged 4 to 6 and 1,378 kcal for a girl, rising to 1,817 kcal and 1,703 kcal by ages 7 to 10.

The heaviest Happy Meal is close to half a 4 to 6 year-old's daily energy, from one sitting. The classic build is a third. The lightest build is around a tenth, which is less than most children eat at breakfast.
For an older child of 7 to 10, even the heaviest box lands at 36 to 39%, which is an ordinary main meal. Age changes the answer more than most of the menu does.
How the four-nugget figure is worked out
The four-piece Chicken McNuggets that come in a Happy Meal are published as part of the meal, not as a standalone item with its own row, so the figure here is derived from the six-piece: 261 kcal for six works out at 43.5 kcal a nugget, and four of those is 174 kcal. Every other nutrient in the four-piece row is scaled the same way.
Two checks say the method holds. McDonald's own published figure for four-piece nuggets is 174 kcal, which matches exactly. And where the same product is listed at two sizes, the scaling is close to linear: two Veggie Dippers are 160 kcal and four are 321, against 320 if you simply double, an error of 0.3%.
One more piece of housekeeping. McDonald's own site does not respond to automated requests from every network, so the underlying figures for this page were retrieved through the Internet Archive's byte-for-byte copies of the same pages. The numbers are McDonald's, not a third party's estimate of them.

A classic Happy Meal at 536 kcal costs more than a Quarter Pounder with Cheese eaten on its own, and more than a Big Mac on its own. It carries far less salt than either, 1.10 g against 2.7 g, because a nugget box is a low-salt main and the burgers are not.
The verdict: for a child under about seven, the classic 536 kcal box is a large meal and the carrot or apple swap brings it down to a sensible one without touching anything the child came for. For an older child, the classic box is a normal lunch, and the one change worth making is the drink, since orange juice adds 22 g of free sugars that milk does not.
Three side notes on the components. Small Fries are 237 kcal against 337 for the medium bag, while chip-shop chips run at 214 kcal per 100 g, so a Happy Meal portion is genuinely small by British standards. The milk bottle at 125 kcal works out at roughly 270 ml, going by the 46 kcal per 100 ml of semi-skimmed milk. And a 250 ml cola would be 103 kcal, all of it free sugars, which is why it is not among the four UK Happy Meal drinks.
